Hello All Good morning,attached is Health card for XY from Wg who is sick and attended Sololo Hospital the day before yesterday and he has been referred to kijabe Hospital for further treatment.
the detailed of his sickness and possible treatment is shown by the attached document.

Congenital atrasia, most likely it is like that, but during the check-ups done by Paul Guyo everything was negative and regular (check his paper files, not the program of DataBase because the weight is not indicated there).
And if it is an intususcezione?!! It would be also easier to handle.
When did the symptoms of not swallowing solid food start? Did they appear gradually over months or suddenly in a few days? I don't know if Kijabe Hospital is able only to do the OGD or also further investigations needed to obtain the right diagnosis and if they are surgically equiped for that particular treatment.
I would transfer him immediately to Kenyatta Hospital in Nairobi, he is covered by national insurance fund and also the bus is not expensive.
In Kenyatta Hospital they will be able to investigate better and maybe even intervene with a simple surgical procedure or prosthesis.
Anyway, even if the boy will not be operated in Kenyatta Hospital, by sure he will be discharged with further investigation done which will help us to get more information here in Italy.
Besides, also TB should be excluded even if extremly unlikely.
If the internal angle part starts to become ischemic or perforates due to the present cortison therapy the patient is lost.
Therefore don't waste time!
Meanwhile, I will take the X-rays to the collegues in our hospital and will let you know their opinion on the matter.

Atrasia congenita, molto probabilmente è così, ma durante il check-up fatto da Paolo Guyo tutto era negativo e regolare (controllare i file di carta, non il programma di database, perché lì il peso non è indicato).
E se si trattasse di un'intususcezione?! Sarebbe anche più facile da gestire.

Non so se al Kijabe Hospital sono in grado di fare le OGD o anche le ulteriori indagini necessarie per ottenere la diagnosi giusta e se sono chirurgicamente attrezzati per quel particolare trattamento .
Io lo trasferirei immediatamente al Kenyatta Hospital di Nairobi, ...

Al Kenyatta Hospital potranno indagare meglio e magari anche intervenire con una semplice procedura chirurgica o protesi.

In ogni caso, anche se il ragazzo non sarà operato all'Ospedale Kenyatta, di sicuro verrà dimesso avendo eseguito ulteriori indagini che ci aiuteranno ad ottenere più consigli dall'Italia.
Inoltre, la TB dovrebbe essere esclusa, anche se estremamente improbabile.
Se la parte di angolo interno inizia a diventare ischemica o perfora causa della attuale terapia cortisonica il paziente è perso.
Quindi non perdete tempo!
Nel frattempo, io mostrerò i raggi X ai colleghi del nostro ospedale e vi farò sapere il loro parere in merito.

The patient has reached Kenyatta hospital.they visited a private doctor who examined the child and adviced on immediate operation.

According to Paul the health of the child is going down and service at kenyatta hospital.

The private doctor estimate the cost at 350,000 to 500,000. I have requested APS. I also adviced Paul to have XY admitted to kenyatta hospital because of it is cheaper.NHIF payment is toward accommodation and some drugs. They do not cover all the cost of treatment.
I will update you as Paul provide more information

Il paziente ha raggiunto il Kenyatta hospital. E' stato visitato da un medico privato che lo ha esaminato e che ha consigliato di operarlo immediatamente.

Secondo Paul la salute del bambino sta andando giù e il servizio al Kenyatta ospedale.

Il medico privato ha stimato un preventivo da 350.000 a 500.000. (3.500-euro / 5.000-euro) Ho fatto richiesta a APS---.

Ho anche avvertito Paul di farlo ricoverare al Kenyatta ospedale dato che lì il conto sarà meno caro. NHIF (Servizio Sanitario Nazionale) paga solo la degenza ed alcuni farmaci. Non copre tutto il costo del trattamento.
Vi aggiornerò appena Paul fornirà maggiori informazioni

thanks ---. APS contacted me today to locate the child but XY was in Nairobi a waiting admission to Kenyatta hospital on 26th september.
I have texted the location of XY, his mother and assisting relative in Nairobi. i hope they will find them and give a supporting hand.
I will provide more information whether APS will provide support

Ho inviato un sms spiegando la situazione di XY, della madre e dell'assistenza relativa (accompagnamento dell'infermiere Paul) a Nairobi. Spero che possano andare a trovarli e poi dare una mano di sostegno.
Mi impegno a fornire ulteriori informazioni se APS--- fornirà supporto

XY was reffered to kenyatta hospital for growth or tumour in the chest. he was supposed to examined and admitted 26th september.

endoscopy done on the tumour showed an advance stage of the tumour.

the doctor advice was that the boy could not survive the operation hence they could not perform the operation on him..

They further adviced to insert a feeding pipe through to the stomach.

they costed the of the pipe at 30,000/= while the the doctor to charge 5,000/= the total fund request is 35,000/= beside transport to come back.

we feel the pipe does not increase the quality of life for tadicha

XY è stato inviato al Kenyatta Hospital per una massa o cancro nel torace. Avrebbe dovuto essere visitato ed ammesso il 26 settembre.

L'endoscopia eseguita ha mostrato un tumore in fase molto avanzata.

Il consiglio medico è stato che il ragazzo non avrebbe potuto sopravvivere all'operazione e dunque non potevano intervenire su di lui..

Hanno inoltre consigliato di inserire un tubo di alimentazione attraverso lo stomaco.

Il costo la del tubo è di 30,000 / = (300-euro), mentre la parcella del medico è 5.000 / = (50-euro) La richiesta è per un costo complessivo di 35.000 / =(350.000-euro). Occorre considerare anche i costi del viaggio per tornare (a Sololo).

Sappiamo che il tubo non aumenta la qualità della vita per XY.

The feeding pipe HAS to be inserted.

It will not increase life expectancy, but, by sure, it will increase the condition to pass away in a less painful and more peaceful way.

Each patient has his own very personal story: his birth, his own life and his death.

Each patient has the right to live his own story in the best possible way.

Our task in every moment is always the same: assisting him.

And that is exactly what we are trying to do every day through Sololo Project.

Therefore, considering this specific case, XY needs to be assisted respecting his right to pass away in the most human and respectable way.

Il tubo di alimentazione deve essere inserito.

Non farà aumentare l'aspettativa di vita, ma, di sicuro, migliorerà le condizioni per una morte meno dolorosa e più serena.

Ogni paziente ha il diritto di vivere la propria storia nel miglior modo possibile.

Il nostro compito in ogni momento è sempre lo stesso: assisterlo.

... even XY has come back home from the hospital safe, with the pipe for feeding, but this morning he seems to be weak but after feeding he will be okey, I took him to waye godha early in the morning, and they will put the drip on,

it is sadening to report that XY died this evening at wayegoda after arrival from Nairobi on 18th october. the burial will take place tommorrow . the project will assist in food
stuffs like rice and Oil and facilitate transport to Wayegoda.
we shall send Gufu Duba and Abdi to participate in the burial.
we pray and wish the family the strength to overcome this trying moment

we went to waye godha for the bariaul of the late tadicha and every thing was okey by that time, the family are okey,tadicha was from a large family and in such family people are crying a lot , particularly girls , and young ladys
but all in all we finish the bariaul by mid day ,so that was how the bariaul went
